Initial Visit

Please complete and sign the New Patient forms sent to you electronically by email from our staff. Kindly arrive 15 minutes before your scheduled appointment. Please bring the following information with you:

  • Referral form if required by insurance
  • List of current prescriptions + over the counter medication
  • Significant medical or surgical history
  • Insurance card and picture ID

At Integrative Behavioral Health Science we try to make it as easy as possible to set up an appointment in the shortest possible time. If for any reason you cannot keep your appointment please notify us within 24 hours before your appointment.

New Patient

Please complete and sign the New Patient forms sent to you electronically by email from our staff. Kindly arrive 15 minutes before your scheduled appointment. Please bring the following information with you:

  • Referral from Primary Care Provider.
  • Hospital discharge papers (if applicable).
  • List of all medication prescription meds and over the counter meds.
  • Insurance card and government issued ID.
  • All relevant lab work and imaging studies within the last 6 months.

Established Patient

  • Please inform us 24hrs prior to your appointment for any cancellations or rescheduling.
  • No call/no show will be assessed $50.00.
  • All phone calls will be returned within 24 hours.
  • Due to a nationwide shortage of several medications your provider will attempt one time to re-send and/or change your medication.

Self-Pay Rates

For patients without insurance or choosing to pay out of pocket, our self-pay rates are as follows:

  • MD Initial Visit: $275.00
  • MD Follow-up Visit: $175.00

A non-refundable $50 retainer fee is collected at the time your appointment is set up. This fee counts towards your deductible or co-pay if the visit is honored. Initial appointments on weekends will be considered NO SHOW if cancelled, and the $50 fee will be incurred.

Insurance Plans and Payment Policy

We accept most insurance plans. Please call to confirm your plan is accepted and to verify your coverage before your visit.

Co-pays and deductibles for office visits are determined by insurance plans and are expected to be paid in full at time of each visit.

We DO NOT accept checks or American Express
