Welcome to Integrative Behavioral Health Sciences

We are glad you are taking this step toward better mental health. Our team is here to support you through every stage of your care — from your very first call to ongoing treatment.

Here is everything you need to know to get started as a new patient.

Step 1 — Contact Our Office

To schedule your first appointment, please call our office during business hours. Our staff will gather some basic information and find the earliest available time that works for you.

Step 2 — Complete Your Intake Forms

Once your appointment is scheduled, our staff will send you new patient intake forms electronically by email. Please complete and sign all forms before your visit.

Step 3 — What to Bring to Your First Appointment

Please arrive 15 minutes before your scheduled appointment time and bring the following:

  • Referral from your Primary Care Provider (if required by insurance)
  • Hospital discharge papers (if applicable)
  • List of all current prescription and over-the-counter medications
  • Insurance card and government-issued photo ID
  • All relevant lab work and imaging studies from the last 6 months

Step 4 — Insurance & Payment

We accept most major insurance plans. Please call our office to confirm your plan is accepted and to verify your coverage before your visit.

Co-pays and deductibles are expected to be paid in full at the time of each visit. We accept MasterCard and Visa debit and credit cards. We do not accept checks or American Express. We DO NOT accept Cash.

  • MD Initial Visit (Self-Pay): $275.00
  • MD Follow-up Visit (Self-Pay): $175.00

Cancellation Policy

Please notify our office at least 24 hours in advance if you need to cancel or reschedule your appointment. Cancellations made less than 24 hours before your visit or on weekends will be considered a NO SHOW and a $50 fee will be incurred.

After-Hours & Emergencies

911: If the situation is unsafe or medically urgent, call 911. Ask for a co-response team and request CIT-trained officers, if available. CIT refers to crisis intervention team training.

988: The nationwide Suicide & Crisis Lifeline offers emotional support but may not be equipped to initiate a local, in-person mobile response.
